Parlodel or Cabergoline should be started when advised by Dr. Miller. It is an oral medication used to reduce the amount of prolactin released by the pituitary.The brand name for bromocriptine is Parlodel® and the brand namefor cabergoline is Dostinex®. . Repeat prolactin levels should be checked after being on the full dose of medication for 4 weeks.

Can be dispensed and taken via Intra muscular injections administered daily. Progesterone may also be administered through a vaginal gel, suppository or in a pill. Progesterone is used to help prepare the uterine lining and to help support the pregnancy.

Prednisone is used to treat positive anti-sperm antibodies and in the treatment of other auto-immunes deficiencies. Prednisone helps to decrease antibody response.

They are used to prevent the risk of infection. Typically Dr. Miller will treat both partners.

16mg tablets are started the day after oocyte (egg) retrieval. They are used to diminish the body’s antibody response.

FSH- An injection given just below the skin (subcutaneous injection) that bypasses the hypothalamus and pituitary glands to directly stimulate follicle growth in the ovaries. Common FSH medications include: Bravelle®, Follistim®, and Gonal-F®.

Human Chorionic Gonadotropin (hCG) also known as Novrel, Ovridel, Pregnyl is given when advised by Dr. Miller to “trigger”. It is given at a specific time to cause the exact timed release (ovulation) of the oocytes (eggs) from the follicles. It allows the oocytes in the follicles to undergo the final maturation process before being released from the ovaries. It also gives us a specific window of when ovulation will occur so that timed intercourse, IUI or Transvaginal Aspiration of Eggs can be performed.

Metformin or Glucophage is a medication that is started after confirming insulin resistance. This is done by a simple fasting blood test that measures the ratio of glucose to insulin. It is taken daily as directed and is discontinued when you become pregnant. It helps to allow for better stimulation with fertility medications. It has also shown to regulate ovulation.

Ganirelix acetate is an injectable competitive gonadotropin-releasing hormone antagonist (GnRH antagonist). It is primarily used in assisted reproduction to control ovulation. The drug works by blocking the action of GnRH upon the pituitary, thus rapidly suppressing the production and action of LH and FSH. Ganirelix is used in fertility treatment to prevent premature ovulation that could result in the harvesting of eggs that are too immature to be used in procedures such as in vitro fertilization

Synthroid is a prescription medication that treats hypothyroidism. The active ingredient in Synthroid is called levothyroxine sodium. It is a synthetic hormone identical to thyroxine – the hormone that the thyroid gland naturally makes. Low levels of thyroid hormone can interfere with the release of an egg from your ovary (ovulation), which impairs fertility. In addition, some of the underlying causes of hypothyroidism — such as certain autoimmune or pituitary disorders — may impair fertility.

Clomiphene stimulates the release of hormones needed to cause ovulation.

Clomiphene therapy is typically used for 5 consecutive days early in the menstrual cycle, for 3 to 6 monthly cycles. It may take several cycles to find the right dose to stimulate ovulation. After that dose is determined, a woman will take the drug for at least 3 more cycles. If she does not become pregnant after 6 cycles, it is unlikely that further clomiphene treatment will be successful.

Why It Is Used

For Women-Clomiphene may be prescribed to:

Stimulate ovulation in a woman who does not ovulate or who ovulates irregularly.
Regulate ovulation in a woman who ovulates irregularly and/or is going to have intrauterine insemination or artificial insemination. Clomiphene is sometimes used together with other medicines and infertility treatments.
For Men- Clomiphene may be used to treat low sperm counts (oligospermia- sperm count < 15x 106/ml).

Most gonadotropin preparations used for infertility treatment are created in a laboratory (called recombinant preparations) and must be injected under the skin to be effective. For most women, a preparation containing only FSH injections is recommended. Women who do not have regular menstrual periods and who have very low levels of LH and FSH require a preparation containing both LH and FSH.
